The story follows Guts, the Black Swordsman, as he travels through a treacherous world of violence, monsters and supernatural powers. Guts, having escaped the events of the Band of the Hawk, continues his journey and protects Casca whose mental state has been severely affected by the traumatic events of her past. Guts is joined by Puck, a small elf that travels with him on his journey. The group eventually enters an area ravaged by a mysterious plague. People are falling ill, strange creatures are appearing, and the whole area seems to be in the grip of an unnatural evil. Soon Guts is caught up in the fighting after running into some people suffering from the strange curse. Guts hacks his way through hordes of enemies with his massive Dragon Slayer sword as he searches for the source of the evil. The journey takes Guts through villages, forests, castles and other perilous places. Guts investigates the plague, meeting many enemies along the way and discovering that the events are tied to a much larger supernatural threat. It gets worse when monster of power starts to show up. Guts has to protect Casca, and at the same time, he has to find a way to understand and stop the mysterious evil. Along the way, Guts meets many of the important characters and villains from the world of Berserk. One of the most dangerous people he meets is Zodd, an apostle and a very strong fighter. Guts’ battles against powerful foes demonstrate the strength of the Dragon Slayer and the power of Guts’ body. It also delves into Guts’s relationship with Casca and his unshakable resolve to protect her in the face of mortal peril. The mysterious plague eventually brings Guts to Balzac, a powerful and dangerous person connected to the events happening around the area. The story is about Balzac’s actions and ambitions as Guts learns more about the supernatural forces behind the plague. Guts will not allow Balzac and the other enemies to continue hurting innocent people. To get to the root of the threat, he has to fight his way through more and more difficult battles. The adventure ends with a series of intense encounters against powerful enemies and supernatural creatures. Guts must depend on his huge sword, other weapons, his physical strength and his Berserk powers to survive. In the final battles, Guts is pushed to his limits as he faces the forces behind the crisis. Having defeated his enemies and stopped the immediate threat, Guts continues on his journey with Casca and Puck. The story takes place in the greater Berserk timeline, around the time after Guts and Puck leave with Casca.

Sword of the Berserk: Guts’ Rage is a 3D hack-and-slash action game. Players control Guts, and fight off large groups of enemies with his huge Dragon Slayer sword. Guts can use a variety of attacks, combos, blocks, dodges, and special moves. Combat has two main states, one of which is the powerful Berserk Rage mode. Other weapons available for players include knives, bombs, a bowgun and Guts' arm cannon. Each enemy has a unique attack pattern and weaknesses. The game includes over 30 types of grotesque enemies. Some sequences also include Quick Time Events, where players must react fast to prompts on the screen. The Berserk meter fills over time through combat. Once Berserk Rage is activated, Guts becomes a lot more powerful for a limited time. Players have to be mindful of their health, their weapons and attacks, especially during big battles.

Start the game and choose a new game. Control Guts and traverse the landscapes of the game. Attack monsters with the Dragon Slayer. Defeat groups of enemies with a combination of attacks.
